These Terms of Use govern your use of the website operated by Ascend Pediatrics, LLC (“Ascend Pediatrics”, “we”, “us”, or “our”) at ascend-pediatrics.com, and your participation in our text messaging program. By using this website or by consenting to receive text messages from us, you agree to these terms. If you do not agree, please do not use the site or enroll in text messaging.

2. No medical advice and no emergencies

The information on this website is provided for general informational purposes only. It is not medical advice and it does not create a physician and patient relationship. Always speak with your child’s provider about any medical question or condition.

Do not use this website or our text messaging program for medical emergencies. If your child has a medical emergency, call 911 or go to the nearest emergency room immediately.

3. Text messaging program

Program description. Ascend Pediatrics, LLC sends text messages to parents and guardians who have given consent. Messages may include appointment reminders and confirmations, follow up on medical records requests, billing notifications, and practice announcements such as seasonal vaccine clinic availability.

• How you enroll. You enroll by checking the text message consent box on the Patient Information Form. The box is optional and is not required to receive care or to complete the form.
• Message frequency. Message frequency varies.
• Cost. Message and data rates may apply. These are charges from your mobile carrier and Ascend Pediatrics does not control them.
• How to opt out. Reply STOP to any message to stop receiving texts. You may also call the office at 931-255-4100 and ask to be removed. After you opt out you will receive one message confirming that you have been unsubscribed.
• How to get help. Reply HELP to any message, or call the office at 931-255-4100.
• Carriers. Mobile carriers are not liable for delayed or undelivered messages. Delivery is not guaranteed and may be affected by your carrier, device, or service coverage.
• Your responsibility. You confirm that you are the account holder or the authorized user of the mobile number you provide, and that you are the parent or legal guardian of the patient. Please tell us promptly if your phone number changes.
• Privacy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt in data and consent are not shared with any third party. See our Privacy Policy for how we handle your information.

4. Use of this website

You agree to use this website only for lawful purposes. You may not attempt to gain unauthorized access to any part of the site, interfere with its operation, or use automated tools to collect information from it.

5. Intellectual property

The content on this website, including text, images, and the Ascend Pediatrics name and logo, is owned by Ascend Pediatrics, LLC or its licensors and is protected by applicable law. You may not reproduce or distribute it without our written permission, except that you may download and print our patient forms for your own use.
